I understand that participation includes supervised swimming and recreational activities. I acknowledge the inherent risks associated with these activities, authorize emergency medical treatment if I cannot be reached, and, to the fullest extent permitted by Alabama law, release and hold harmless People of God Community Church, its staff, and volunteers from liability for ordinary negligence related to my child's participation.
